ditto to narcus choice, that thing was utterly awe-inspiring. I remember actually standing in the control room for all of five minutes, staring at those things and thinking fearful thoughts.
The Gargant in HL was also pretty scary.
Scariest is when they completely take you by surprise though, and I find that because you experience a few moments of complete lack of thought, it really doesn''t matter what came at you.
I remember playing some Aliens-like game, where I was running around, and then out of no-where, about fifteen of the medium-level aliens jumped at me from the ceiling (side-scroller), I wasn''t really in danger, but I was jumpy as all hell for a good half hour.

Good call on the HL pit-beast. Scary and unique and interesting, it gave that entire area great atmosphere.
One I would mention in a boss in Ogre battle, Gares the Black Knight. In the Japanese version he attacks by making a giant flaming pentacle rise up from the floor. (In the US they changed it to be a circle...) And he has two dragons protecting him, really cool the first time you fight him. Actually almost every enemy in that game is very well done, even though most are typical fantasy creatures they don''t feel generic and boring, due to the nice animation touches and other TLC that went into them.

Im quite fond of the Fiend from Quake, as well as CyberDemon from Doom, and the Cacodemon's Hiss always got me edgy.
Any monster that charges at you without any regard for its own life, and is relatively hard to kill, is good. ie: Knights from Quake, they charge at you just to cut off your head, and they take quite a few shotgun blasts to kill, in close quarters, they scare you. Also, the Headless Serious Sam Bombers. "AAAAAAAAAHHHHH!" BOOM! Having hundreds of those charge at ya sucked balls. oh - and the headcrabs from Hl. Not hard to kill, but damn annoying in groups.

I think clowns are really creepy. Seriously From all the monsters of the world i think clowns are the most frightening. Maybe it''s because they have always such an unnatural smile on their face and such unhuman clothes.
