how about IcoFX?
I don't see any information about whether or not it can be used in commercial apps(I failed to read the license :D during install), but I'd imagine that it's free for commercial use too.

Gives you a file with animated version also. I use it all the time :)

When you also want to include an alpha channel in your icon (translucency as opposed to simple on/off transparency), I'd recommend using your favorite graphics program to create a PNG file with alpha and converting it through @icon sushi (or edit it in @icon sushi directly). This also enables you to easily combine many differen icon formats into the same file.

I just got IcoFX. This is a very good program. I imported a bitmap with an alpha channel, and it only took me minutes to make a high-quality icon with high functionality.

Note: If you use IcoFx, you must disable Vista png compression or your icons will not work in Visual Studio (at least for VC++2005 Express)

I use Gimp. It works well enough for my purposes, and is able to handle transparency.
