Telnet negotiation

I''ve checked various RFCs and guides online but I can''t find any simple answers here. Is it necessary for a client or server to remember which DO/DONT/WILL/WONT command it initiated, so that it can avoid resending that command in response to one from the other side? How do you know when not to reply to a command - ie. how do you avoid getting into a DO - WILL - DO - WILL loop? Are there any rules as to who (client or server) should initiate negotiation for a given option? And what is the procedure to follow if both sides send an option request at the same time, leading them to both believe they initiated the negotiation for that request?
