Narcoleptic
You play as the Earth Guardian Father Time. You''ve gotten old over the years and have developed a serious case of Narcolepsy. If you fall asleep while on the job, the world will break apart and fall into chaos. So to prevent the destruction of the world you must destroy the invading "Rapid Eye Movements" (REMS) and cataplexy attacks that make you sleepy, in a dance dance revolution style shooting/dancing game!
Survive all of your work shifts, stay awake, and save the world!!

I may still have somewhere, a proto-design for "Sloth" - a game where the hero is a teenager who''s house-sitting, and the aim is to do as little as possible and get as much money as possible at the end. So, for example, watering the plant takes repeated effort, while if you let it die unreplaced, you take a serious cash hit when the owners return - if you replace it, you have to put in effort to get to the shops, and more effort to get a closer match... Or eating - requires effort, but the hungrier you get, the more effort everything else takes. And so on...

heh, the "Sloth" game sounds like it could make a fun puzzle.
It could be neat to have a "real world" and "dream world(s)" in a game.. a touch of psychic power could make them affect eachother somehow. Perhaps you''d have abilities in the dream worlds you wouldn''t have in other worlds, that could somehow set up situations that *were* solvable in the real world...
I have a couple of designs cooking around in my head. Without going on and on about them the summaries are: -Survival Horror Game, groundbreaking in it''s depiction of ''mature'' material. -Post Apocalyptic Urban Combat Game, explains itself. -Space Empire Strategy Game, it''s been done before, but I have some things I''d like to see done.

This is a two-player ONLY network game. Probably a platformer of some sort, 2D or 3D.
Player 1: You find yourself in a strange land. Bobo the monkey king gives you a quest: visit 5 strange lands and find the mighty relic hidden in each! On your path you will have to fight (or flee from) all sorts of strange and bizarre creatures!
Player 2: You wake up in the middle of the night by some strange noises - your friend (Player 1...) is sleepwalking, and mumbling about some damn monkey?! You can't seem to wake him up, so you better help him survive - your friend is headed right out in the street and into the city! Make sure he does not get hit by cars, get mugged, fall down from rooftops etc.
The idea is to have the both worlds have roughly the same shape and geometry, but having really different contents. Player 1 is playing his wacky dream game, trying to accomplish his missions. But what he sees is probably something completely different in the Real World, or maybe nothing at all.
Examples of situations: DM (dream world): On his way through a beautiful meadow (with flowers that sound like car horns...?!), a beautiful horse comes along and lets Player 1 ride. As the horse gallops away, Player 1 tries to jump and catch those gold coins floating in the air! RW (real world): Player 1 is headed into the street! Player 2 decides to carry him, but it's not that easy when Player 1 insists of doing crazy jumps forth and back!
DM: Player 1 is on a Carribean island, looking for a famous pirate treasure. After doing some digging, he finds gold bars! RW: Player 2 is risking his life as he's navigating Player 1 through a dangerous construction site. Player 1 finds some bricks and starts shouting in pirate jargon...?
Hmm... now when I read my post, I think about Don Quijote...

UPDATE: rileyriley, I didn't see your post earlier, seems we were both onto the Real World / Dream World(s) idea...
Another approach would be to infiltrate other (AI-controlled) people's dreams at night, affecting how they would react to events during the next day. Maybe you were reaching into their subconciousness, which would not make them aware of their dreams and how they are affected.

how about something a little more far-out:
a social adventure... the player plays a being who can interact with NPCs only when they sleep, giving them cues, triggers and instructions, subtly changing their behaviors by creating or changing dreams for them. After the night is over you see them interact in a sims-like environment, seeing the results of your (presumably evil) deeds. The goals are irrelevant, the change in the NPCs personalities is gradual and interdependent (i.e. you let the daugher dream that nobody loves her, causing her to run away from home, thereby pushing the mother into depression.. a few dreams about the death of the daugher push her further down into madness until she finally goes and axes the husband... who happened to be you "objective").
