Punishment is important, let''s get back to the monsters example:
Normally if he fails to kill the monster he get killed himself.
Let''s asume that deaths will be punished harsely in a game, this
will cause that if a normal player is killed serveral times he won''t enjoy the game. So he must have another way of failing and be punished less hard.
He must have the abilty to escape the monster, but be punsished in the form of injuries or by broken material or equipment loss.
Like his armour has taken bad damage etc.
Why do players need some punsihedment casue the greater the possible penalty the greater the sense of achievement.
Reward is only 50% of the pleasure. So you better find a balance.

well spoken!!
punishing a player for failure by making the game much harder is a safeway to frustration, imo the difficulty level shouldn''t be increased, my idea of punishing, is putting a player in a situation where he doesn''t want to be, for example being caught and locked in a cell, losing an essential item or weapon, things like that, where you are given a tricky task to do to get you back to you position before you flopped, most important is that the player feels a loss, and tries to avoid making mistakes again or just play the best he can.
i believe that this can be achieved correctly if the game;''s mechanism and rules are designed around such ''system''
