Cover letter as the email body or as an attachment?

Quick question: When emailing an application to a job, should the cover letter be the email body, or should it be an attachment? Or both? If attachment only, then what should be in the email body? My thought is to have it as the email body, and additionally attach it to the email, along with the resume.
Just put it as the email body. It's unlikely that anybody will read it regardless.

As a side note, do I need to really need to provide a header with the company address? It seems unnecessary for an online application. Also, it doesn't help that so many websites make it impossible to find their address.

Are there any notable differences between game company cover letters versus normal cover letters? Should I still be addressing it "To whom it may concern"? (or maybe, "Dear hiring manager:")

Use your initiative, call them up asking who to address it to. This shows that you have given some thought and there is a very slim off chance that this is a +1.

I agree with Magic Man. Go the extra few yards, phone the company and find out who the HR person is and then address the email to them by name.

If they specifically requested a cover letter, I'll attach it. Otherwise, I put it as the body of my email.
