There will always be somebody who finds whatever you do offensive, whether reasonably or not.
Also, with situations as opposed to individual words, I think theres a difference between the game being offensive to the player, and game characters being offensive to the player and their character. It can be an acceptable part of the game for the player to be offended by the characters in game.
For example, in Morrowind, when the guards say "We're watching you... scum!". I actually enjoyed this because it made the game feel a lot more immersive. Especially since it was quite well timed when I was walking past for it to seem like they were adding 'Scum' when they thought I would be just out of earshot, or intentionally for me just to hear it quietly.
But I think the best way to avoid offending so many people would be to make sure the issues in the game are dealt with maturely, without seeming to be a naive interpretation, or placing too much of any particular slant on things.
