Original post by Michalson Weight wouldn't be very realistic in a medieval RPG - up until a few hundred years ago being fat was a sign of wealth. No one else could afford to eat in the quantity, or quality (variety of foods i.e. meat, vegetables, fruit) to get overweight. The back breaking labor also helped keep everyone who had seconds thin.
Well, unlike in real life, in video games it is actually easier to get thin than to get fat. I mean, show me a gamer that prefer his character sitting on it's ass and eating than running around, fighting, and do other phyisical activities.

Fable! Some parts of the game cannot be reached without maintaining a fat character
It would be interesting to have characters with the build of John Rhys Davies (sp?) while not being a Dwarf. It would be interesting to have different types of armor, for the tall, muscular characters and the shorter, more stout ones. So you end up having to have armor tailored for the character or possibly not being able to wear armor because it just won't fit your character.
To some extent, it would just be an annoyance, but in other ways, it would add a lot of flavor to the game.
