How to spice up MMORPG's?

Turbine is trying to create 'impression' of real-time combat (it's probably more accurate than actually call it 'real time' as it's to some degree faked) for their Dungeons&Dragons Online. If you run a search on DDO forums, you'll find a few posts from developers detailing what tricks they use to achieve this.
