Basically I want to know what linux does if I chosse: ''always keep link up'' as opposed to ''specifying timeout''. In both cases I have to use adsl-start to connect.
I want to only be in the net when I enter adsl-start and not be in the net when I type adsl-stop.(I pay per hour). Which should I use now? I mean they seem to do the same for me.

''Always keep link up'' and specifying a timeout indicate what to do by default if you do not stop the connection manually. If you type adsl-stop, it will terminate the connection regardless of how you set the above options.
Since you pay by the hour, you would probably want to not keep the link up, and specify a short timeout period (say 15 minutes). As well, you''ll want to get into the habit of manually shutting down the connection. This gives you control over your connection, but protects you if you forget to run adsl-stop.
