guidelines what as models and what as map?

Hi, are there any guidelines wich stuff you create as a model and wich as part of the map? in this particular case im talking about a building. Do i make it as .md2 separate or include it in the .bsp? or are entities that can be moved preferably a modle and entities wich are static and never change as a map? then in wich format someone usually stores a object for a collection of "mapobjects"? maybe i should move to milkshape and drop this .psb .md2 sutff cmon give some fool some suggestions...
I would say all entities as models (so the map doesn''t contain entities, which is most logical and easy to use with classes, a class world and a class entity). Take a look at the half-life, quake and unreal engines for 3 good examples.
